Pursuant to the Law on Government Organization dated June 19, 2015;
Pursuant to the Law on amendments to the Law on Governmental Organization and the Law on Organization of Local Governments dated November 22, 2019;
Pursuant to the Law on Prevention and Control of Infectious Diseases dated November 21, 2007;
Pursuant to the Government's Decree No. 101/2010/ND-CP, which elaborates some Articles on quarantine, quarantine enforcement and epidemic control of the Law on Prevention and treatment of infectious diseases;
Pursuant to Decision No. 2194/QD-BCDQG dated May 27, 2020 of National Steering Committee for Covid-19 Control on promulgation of the Guidelines for Covid-19 Prevention And Infection Risk Assessment in Workplaces and Worker Dormitories”;

At the request of the Director of Department of Culture and Sports in Report No. 4163/TTr-SCT dated September 15, 2021 and opinions of the Director of the Department of Health in Official Dispatch No. 6626/SYT-NVY dated September 15, 2021,
HEREBY DECIDES:
Article 1. The Criteria for Evaluation of Safety and Prevention Against Covid-19 in physical activity and sports entities (enterprises, public sector entities, sports clubs, stadiums, fitness centers) in Ho Chi Minh City.
Article 2. This Decision comes into force from the day on which it is signed and supersedes the Set of Criteria for Evaluation of Safety and Prevention Against Covid-19 of physical activity and sports entities (enterprises, public sector entities, sports clubs, stadiums, fitness centers) in Ho Chi Minh City in Decision No. 1397/QD-BCD dated April 25, 2020 of Ho Chi Minh City Steering Committee for Covid-19.
Depending on circumstances, Ho Chi Minh City Steering Committee for Covid-19 may consider revising the criteria in conformity with the pandemic situation in Ho Chi Minh City.
Article 3. Chief of Office of the People’s Committee of Ho Chi Minh City, Director of Department of Health, Director of Department of Culture and Sports, heads of other Departments and authorities of the city, the President of the People’s Committees of districts and Thu Duc City, Presidents of the People’s Committees of wards, communes and commune-level towns, relevant organizations and individuals are responsible for the implementation of this Decision./.

FOR
EVALUATION OF SAFETY AND PREVENTION AGAINST COVID-19 OF PHYSICAL ACTIVITY AND
SPORTS ENTITIES (ENTERPRISES, PUBLIC SECTOR ENTITIES, SPORTS CLUBS, STADIUMS,
FITNESS CENTERS) IN HO CHI MINH CITY
(Issued together with Decision No. 3325/QD-BCD dated September 15,
2021 of HCMC Steering Committee for Covid-19 Control)
I. SET OF CRITERIA: 10 criteria
A. Compulsory criteria
1. Criterion 1: employees (employees, instructors, service staff, etc. employed by the physical activity and sports entity, regular employees) shall satisfy the conditions below: having received 2 doses of vaccine against COVID-19 or having fully recovered from COVID-19 in the past 6 months; taking periodic antigen tests as prescribed by the Department of Health, and following safety protocols in working environment:
- 100% of employees satisfy all conditions: Passed;
- Any of the employees fails to satisfy all conditions: Failed.
2. Criterion 2: participants in physical activity and sports shall satisfy the conditions below: having received 2 doses of vaccine against COVID-19 or having fully recovered from COVID-19 in the past 6 months.

- Any of the participants in physical activity and sports fails to satisfy all conditions: Failed.
3. Criterion 3: collect customer’s information (via QR codes, health declarations in person or online), monitor and update participants come for practices on a daily basis.
- The tasks are performed: Passed;
- The tasks are not performed: Failed.
4. Criterion 4: physical activity and sports instructors, participants in physical activity and sports and service staff shall comply with 5K rules while doing activities and making direct contact with others:
- The tasks are performed: Passed;
- The tasks are not performed: Failed.
5. Criterion 5: prepare an area for taking body temperature at entrances, prepare sanitizers and hand-washing soap and clean water at the practice areas. Disinfect the surface of equipment (including personal devices), disinfect practice areas 2 times a day.

6. Criterion 6: conditions for distance, safety density, queuing at service and practice areas, at least one of the following conditions are satisfied: (air-conditioners are not used, open doors and windows for ventilation).
a) In respect to outdoor physical activity and sports: at least 10m2 per person and a minimum distance of 2 m between two people.
b) In respect to physical activity and sports in: centers; indoor stadiums; sports venues; sports clubs: at least 8m2 per person and a minimum distance of 2 m between two people.
c) In respect to physical activity and sports in fitness centers: at least 6m2 per person and a minimum distance of 2 m between two people.
- All conditions are satisfied: Passed;
- Any of the conditions is not satisfied: Failed.
B. Optional criteria
7. Criterion 7: There is a plan for entrance to and exit from the physical activity and sports areas. There is a diagram for separate entrance and exit and the diagram is implemented, one-direction movement is carried out.

8. Criterion 8: the physical activity and sports entity disseminates and inform pandemic prevention and control measures to instructors, participants in physical activity and sports and service staff as recommended by the health authorities and competent authorities in various and diversified forms:
- The tasks are performed: Passed;
- The tasks are not performed: Failed.
9. Criterion 9: publish contacts of on-site staff that are responsible for COVID-19 prevention and control and coordination with the competent authorities.
- The tasks are performed: Passed;
- The tasks are not performed: Failed.
10. Criterion 10: there is a plan for administration, cooperation, and inspection of COVID-19 prevention and control on site. There is a plan for Covid-19 prevention and control according to instructions of health authorities, including the plan for handling suspected and confirmed Covid-19 cases; plan for closing, reopening after safety and prevention against Covid-19 are ensured; The plan shall be revised according to reality and evaluation result. Inform the local authority for inspection.
- A plan is available and implemented: Passed;
- There is no plan or the plan is not implemented: Failed.

1. At least 8 criteria are satisfied and no compulsory criterion is unsatisfied: the entity ensures safety against COVID-19 and is allowed to operate. With regard to any of unsatisfied criteria, remedial measures shall be taken within 48 hours.
2. Less than 8 criteria are satisfied and any compulsory criterion is unsatisfied: the entity does not ensure safety against COVID-19 and must suspend operation; and quickly take remedial measures as guided for resumption of operation./.
