Decision No. 1055/QD-BHXH of October 02, 2013, on promulgating model of health insurance card

DECISION

ON PROMULGATING MODEL OF HEALTH INSURANCE CARD

GENERAL DIRECTOR OF THE VIETNAM SOCIAL INSURANCE

Pursuant to the Law on health insurance No. 25/2008/QH12 dated November 14, 2008;

Pursuant to the Government’s Decree No.  94/2008/ND-CP dated August 22, 2008, defining the functions, tasks, powers and organizational structure of Vietnam social insurance; Pursuant to the Government’s Decree No. 116/2011/ND-CP dated December 14, 2011, amending and supplementing a number of Articles of the Government’s Decree No.  94/2008/ND-CP dated August 22, 2008, defining the functions, tasks, powers and organizational structure of Vietnam social insurance;

Pursuant to the Government’s Decree No. 62/2009/ND-CP dated July 27, 2009, detailing and guiding a number of articles of the law on health insurance;

Pursuant to the Joint Circular No. 09/2009/TTLT-BYT-TC dated August 14, 2009, of the Ministry of Health, and the Ministry of Finance, guiding implementation of health insurance;

At the proposal of Head of division of issuing insurance books and cards,

Article 2. Prints of health insurance card have size, material and form as follows:

1. Size:Length of 98 mm, width of 66 mm, according to frame of outer edge of card (sufficient for printing 08 health insurance card prints on 01 page, size A4).

2. Material:White paper, weight of 200g/m², ensured on durability, and gloss able to print picture of persons participating in health insurance. 

3. Form:White background, in the middle, print a dim symbol of Vietnam Social Insurance with cobalt blue and circles spreading around, color gradually reduced, distance from center of symbol of Vietnam social insurance to the outer circle is 12 mm; there is a frame for hemming printed by double line with cobalt blue.

3.1. The front side:

3.1.1. At adjacent to the left margin of health insurance card:

- On the top, print a symbol of Vietnam social insurance with cobalt blue, diameter of 10mm, tamper-proof reflective material is used.

- Next lower position for affixing or printing picture of persons participating in health Insurance.

- At the bottom, it is position to print barcode.

- On the top, there is phrase “VIETNAM SOCIAL INSURANCE”, the underline with cobalt blue, font .VntimeH, bolt, size 10.

- Next lower position, there is phrase “HEALTH INSURANCE CARD”, red, font .VntimeH, bolt, size 12.

- Along to hem of right margin of health insurance card, print “serial number” of the print of health insurance card, red, font .Vntime, size of 10 to 12, including 10 characters, in which:02 first characters is number (from 01 to 99) suitable with provincial code prescribed in Decision No.  124/2004/QD-TTg dated July 08, 2004, of the Prime Minister, particularly, 02 first characters of health insurance prints issued by social insurance agencies of the Ministry of National Defense will be 97, issued by social insurance agencies of People’s Pubic Security will be 98, issued by social insurance agencies of the Cipher Board of Government will be 99. And 8 following characters will be numbers (natural number from 00000001 to 99999999).

- Unit seals of social insurance agencies of provinces and central-affiliated cities (hereinafter collectively referred to as provincial social insurance agencies) are printed on health insurance card prints, diameter of 21mm, red, tamper-proof reflective material is used.Position from internal edge of frame at right of card to outer edge of seal will be 28 mm, from internal edge of frame hem at below of card to outer edge of seal will be 2mm.

3.2. The back side:

- On the top, print phrase “NOTES” which is black, font .VntimeH, size 12.

- At next lower position, print the phrases which are black, font .Vntime, italic, size 10, with content as follows:

 “1. Health insurance card is grounds to define the person named in card entitled to enjoy rights and benefits on health insurance.

2. Each person participating in health insurance is issued a card and a code does not change during participating in health insurance.

4. To preserve carefully, not let other person borrow card and it is necessary to remember the code on health insurance card.

5. In case of loss or damage, when re-issuing or altering card, pay charge according to regulations.” 

Article 3. Information printed on health insurance card prints when issuing to users include:

1. Items for managing persons participating in health insurance.

Code of health insurance card:Recording the card code of person participating in health insurance issued according to current regulations of Vietnam social insurance.

1.2. Full name:Full name of person who is issued Health insurance card, by uppercase, under the list of registration for participating in health insurance.

1.3. Gender:Male or female under the list of registration for participating in health insurance.

1.4. Date of birth:date of birth of person participating in health insurance (under dossier of managing social insurance and health insurance revenues).Especially for persons having no date of birth, inscribe the birth year.

1.5. Address:Address of residential place (permanent residence or temporary residence) or address of unit participating in health insurance. For children under 06 years, inscribe address of mother or father or guardian.

1.7. Use time:From date to date.

1.8. Time to begin enjoying rights and benefits of high-technical services, big cost or time to enjoy rights and benefits of drugs for cancer treatment, anti-rejection drugs (not in the list prescribed by the Ministry of Health but permitted circulation in Vietnam according to designation of medical examination and treatment establishments): “level … from date ……………”, in which:

- “Level 1 from date ……….”: ": meaning time to begin enjoying big rights and benefits for objects voluntarily participating in health insurance continuously for full 180 days or more; for other objects: let blank.

- “Level 2 from date ……….”: ": Meaning time to begin enjoying rights and benefits of drugs for cancer treatment, anti-rejection drugs, for persons participating in health insurance continuously for full 36 months or more (on first day of seventh month). Particularly children under 6 years, time to begin is the beginning day of participating in health insurance; objects under management of the Ministry of National Defense, the Ministry of Public Security, Cipher Board of Government , are entitled to enjoy regime of medical examination and treatment without charges according to regulations, but now they have been retired, transferring sector where they are participating in health insurance,  time to begin enjoying rights and benefits will be date of retirement or transfer of sector. 

Particularly children under 6 years, apart from above items, add item “Parents”: Full name of father or mother of child. In case there is not father and mother, inscribe full name of guardian.

2. Position to affix picture:A blank frame, single line, black, size 20mm x 30mm. Particularly children under 6 years, it is not required picture, information of children will be move to left margin (position for printing or affixing picture) so as to make a well-proportioned health insurance card print.

3. Position to print barcode: Barcode printed on health insurance card is barcode of two directions, containing information on card and character chain for examination, barcode is printed in the blank space adjacent to left margin of health insurance card and at below of position to print or affix picture of person participating in health insurance. 

Article 4. The Information Center of Vietnam social insurance shall base on contents specified in Article 3 of this Decision, adjust the software to be suitable to items printed on health insurance card and requirements on management.

Article 5. This Decision takes effect on January 01, 2014, replace Decision No. 653/QD-BHXH dated June 05, 2009, Decision No. 1754/QD-BHXH, dated December 20, 2012, of General Director of Vietnam social insurance. Documents which are issued by Vietnam social insurance in previous time and contrary to this Decision are annulled.

1. Models of health insurance card prints as prescribed in Decision No. 653/QD-BHXH, of General Director of Vietnam social insurance which are inventory till December 31, 2013 will be continued using for issue of health insurance card to objects participating in health insurance.

2. Health insurance cards issued before the effective date of this Decision are valid until the expired day of use inscribed on health insurance card.

Article 7. Head of division of issuing insurance books and cards, the Chief of office, heads of units affiliated Vietnam social insurance, directors of social insurance agencies of provinces, centrally-run cities, and directors of social insurance agencies of the Ministry of National Defense, the Ministry of Public Security, and Cipher Board of Government shall implement this Decision.
