VỀ VIỆC TRIỂN KHAI ÁP DỤNG NGUYÊN TẮC THỰC HÀNH TỐT BẢO QUẢN THUỐC

Căn cứ Luật bảo vệ sức khỏe nhân dân ngày 11/7/1989 và Điều lệ thuốc phòng bệnh, chữa bệnh ban hành kèm theo Nghị định số 23/HĐBT ngày 24/01/1991 của Hội đồng Bộ trưởng (nay là Chính phủ);
Căn cứ Pháp lệnh chất lượng hàng hóa ngày 24/12/1999;
Căn cứ Nghị định số 68/CP ngày 11/10/1993 của Chính phủ quy định chức năng nhiệm vụ, quyền hạn và tổ chức bộ máy Bộ Y tế;
Căn cứ Nghị định số 86/CP ngày 08/12/1995 của Chính phủ quy định phân công trách nhiệm quản lý Nhà nước về chất lượng hàng hoá;
Xét đề nghị của Cục trưởng Cục Quản lý Dược Việt Nam,
QUYẾT ĐỊNH
Điều 1. Triển khai áp dụng nguyên tắc “Thực hành tốt bảo quản thuốc” ban hành kèm theo Quyết định này ở tất cả các đơn vị sản xuất, kinh doanh, xuất nhập khẩu, buôn bán, tồn trữ thuốc, kinh doanh dịch vụ kho bảo quản thuốc, khoa dược bệnh viện, viện nghiên cứu và trung tâm y tế.
Điều 2. Quyết định này có hiệu lực sau 15 ngày kể từ ngày ký, ban hành.
Điều 3. Ông Cục trưởng Cục Quản lý dược Việt Nam chịu trách nhiệm hướng dẫn, kiểm tra và theo dõi việc triển khai áp dụng nguyên tắc “Thực hành tốt bảo quản thuốc”.
Điều 4. Các Ông, Bà: Chánh văn phòng, Chánh thanh tra Bộ Y tế, Cục trưởng Cục Quản lý dược Việt Nam, Thủ trưởng các đơn vị trực thuộc Bộ, Giám đốc Sở Y tế các tỉnh, thành phố trực thuộc Trung ương, Thủ trưởng Y tế các ngành chịu trách nhiệm thi hành Quyết định này.

“THỰC HÀNH TỐT BẢO QUẢN THUỐC”

Để đảm bảo cung cấp thuốc có chất lượng đến tay người sử dụng đòi hỏi phải thực hiện tốt tất cả các giai đoạn liên quan đến sản xuất, bảo quản, tồn trữ, lưu thông phân phối thuốc.
Thực hành tốt bảo quản thuốc (tiếng Anh: Good Storage Practices, viết tắt : GSP) là các biện pháp đặc biệt, phù hợp cho việc bảo quản và vận chuyển nguyên liệu, sản phẩm ở tất cả các giai đoạn sản xuất, bảo quản, tồn trữ, vận chuyển và phân phối thuốc để đảm bảo cho thuốc có chất lượng đã định khi đến tay người tiêu dùng.
Văn bản này qui định các nguyên tắc cơ bản, các hướng dẫn chung về “Thực hành tốt bảo quản thuốc”. Tuy nhiên, các nguyên tắc, hướng dẫn này có thể được điều chỉnh để đáp ứng các yêu cầu cụ thể riêng biệt, nhưng vẫn đảm bảo thuốc có chất lượng đã định.
Các nguyên tắc “Thực hành tốt bảo quản thuốc” được áp dụng cho các nhà sản xuất, xuất khẩu, nhập khẩu, buôn bán, tồn trữ thuốc, khoa dược bệnh viện, viện nghiên cứu và trung tâm y tế.
Các từ ngữ được sử dụng trong bản nguyên tắc này được hiểu như sau:
2.1 Bảo quản thuốc: là việc cất giữ an toàn các thuốc, nguyên liệu, bao gồm cả việc đưa vào sử dụng và duy trì đầy đủ các hệ thống hồ sơ tài liệu phù hợp, kể cả các giấy biên nhận và phiếu xuất.
2.2 Thuốc là những sản phẩm có nguồn gốc từ động vật, thực vật, khoáng vật, hóa dược hay sinh học được bào chế để dùng cho người, nhằm :
- Phòng bệnh, chữa bệnh.
- Phục hồi, điều chỉnh chức năng cơ thể.
- Làm giảm triệu chứng bệnh.
- Chẩn đoán bệnh.
- Phục hồi hoặc nâng cao sức khoẻ.
- Làm mất cảm giác một bộ phận hay toàn thân.
- Làm ảnh hưởng quá trình sinh sản.
- Làm thay đổi hình dáng cơ thể.
2.3 Nguyên liệu là các chất có hoạt tính hay không có hoạt tính, có biến đổi hay không bị biến đổi được sử dụng trong quá trình sản xuất thuốc. Bao gồm hoạt chất, tá dược, thuốc thử, dung môi, sản phẩm trung gian, bao bì đóng gói và nhãn thuốc.
2.4 Hoạt chất là một chất hoặc hỗn hợp các chất có hoạt tính dược học được sử dụng trong sản xuất thuốc.
2.5 Tá dược là các chất, không phải là hoạt chất, đã được đánh giá đầy đủ về độ an toàn và được đưa vào hệ phân bố thuốc.
2.6 Bao bì đóng gói là mọi vật liệu được sử dụng trong việc đóng gói sản phẩm, loại trừ công-te-nơ được sử dụng để đựng sản phẩm, chuyên chở bằng các loại phương tiện vận tải khác nhau mà không phải xếp dỡ hàng hoá bên trong trước khi đến nơi nhận.
2.7 Bán thành phẩm là nguyên liệu đã được xử lý một phần, và phải trải qua các xử lý tiếp theo trước khi trở thành thành phẩm.
2.8 Thành phẩm là thuốc đã trải qua tất cả các giai đoạn của quá trình sản xuất, bao gồm cả giai đoạn đóng gói.
2.9 Ngày kiểm tra lại là ngày mà thuốc, nguyên liệu cần phải được kiểm tra, đánh giá lại xem có đạt tiêu chuẩn chất lượng theo qui định không.
2.10 Nhãn là bản in, hình vẽ, hình ảnh, dấu hiệu được in chìm, in nổi trực tiếp trên bao bì thương phẩm hoặc được dán, gắn chắc chắn trên bao bì thương phẩm của thuốc để thể hiện các thông tin cần thiết và chủ yếu về thuốc đó, giúp người dùng lựa chọn và sử dụng đúng thuốc, và làm căn cứ để các cơ quan chức năng thực hiện kiểm tra, giám sát, quản lý.
Nhãn bao gồm tất cả các nhãn và các phần in, viết hoặc hình hoạ trên bao bì trung gian của sản phẩm hoặc trên bao bì, vỏ hộp chứa đựng sản phẩm đó, loại trừ công-te-nơ.
2.11 Biệt trữ: là tình trạng thuốc, nguyên liệu được để riêng biệt, trong một khu vực cách ly hoặc bằng biện pháp hành chính để chờ quyết định xử lý huỷ bỏ hoặc cho phép nhập kho hoặc xuất kho cho bào chế, đóng gói hoặc phân phối.
2.12 FIFO là từ viết tắt của tiếng Anh "First In/First Out", nghĩa là "nhập trước- xuất trước".
FEFO là từ viết tắt của tiếng Anh "First Expired/First Out", nghĩa là "hết hạn dùng trước- xuất trước".
2.13 Tạp nhiễm là việc xuất hiện một cách không mong muốn các tạp chất có bản chất hoá học hoặc vi sinh vật, hoặc các vật ngoại lai vào trong nguyên liệu, bán thành phẩm, thành phẩm thuốc trong quá trình sản xuất, lấy mẫu, đóng gói, đóng gói lại, bảo quản và vận chuyển.
2.14 Nhiễm chéo là việc tạp nhiễm của nguyên liệu, sản phẩm trung gian hoặc thành phẩm thuốc với nguyên liệu hoặc thuốc khác trong quá trình sản xuất, bảo quản và vận chuyển.
1.1. Theo qui mô của đơn vị, kho thuốc phải có đủ nhân viên, có trình độ phù hợp với công việc được giao làm việc tại khu vực kho. Mọi nhân viên phải thường xuyên được đào tạo về “Thực hành tốt bảo quản thuốc”, về kỹ năng chuyên môn và phải được qui định rõ trách nhiệm, công việc của từng người bằng văn bản.
1.2. Các cán bộ chủ chốt của kho có chức năng giám sát, kiểm tra, cần phải trung thực, có những hiểu biết, kinh nghiệm cần thiết và phải có trình độ nghề nghiệp và kỹ thuật phù hợp với nhiệm vụ được giao, đáp ứng các qui định của Nhà nước.
1.3. Thủ kho phải là người có trình độ hiểu biết cần thiết về dược, về nghiệp vụ bảo quản: phương pháp bảo quản, phương pháp quản lý sổ sách theo dõi xuất nhập, chất lượng thuốc...
1.4. Thủ kho phải có trình độ tối thiểu là dược sĩ trung học đối với các cơ sở sản xuất, bán buôn thuốc tân dược. Đối với cơ sở sản xuất, bán buôn thuốc y học cổ truyền, dược liệu, thủ kho phải có trình độ tối thiểu là lương dược hoặc dược sĩ trung học.
1.5. Thủ kho thuốc độc, thuốc gây nghiện và thuốc hướng tâm thần phải đáp ứng được đúng các qui định của pháp luật có liên quan.
1.6. Thủ kho phải thường xuyên được đào tạo cập nhật những qui định mới của nhà nước về bảo quản, quản lý thuốc, các phương pháp, tiến bộ khoa học kỹ thuật được áp dụng trong bảo quản thuốc.
Nhà kho phải được thiết kế, xây dựng, trang bị, sửa chữa và duy tu một cách hệ thống sao cho có thể bảo vệ thuốc, nguyên liệu tránh được các ảnh hưởng bất lợi có thể có, như: sự thay đổi nhiệt độ và độ ẩm, chất thải và mùi, các động vật, sâu bọ, côn trùng, đảm bảo thuốc có chất lượng đã định.
2.1 Địa điểm:
Kho phải được xây dựng ở nơi cao ráo, an toàn, phải có hệ thống cống rãnh thoát nước, để đảm bảo thuốc, nguyên liệu tránh được ảnh hưởng của nước ngầm, mưa lớn, và lũ lụt..
Kho phải có một địa chỉ xác định, nằm ở nơi thuận tiện cho việc xuất nhập, vận chuyển, bảo vệ.
2.2 Thiết kế, xây dựng:
a- Kho phải đủ rộng, và khi cần thiết, cần phải có sự phân cách giữa các khu vực sao cho có thể bảo đảm việc bảo quản cách ly từng loại thuốc, từng lô hàng theo yêu cầu.
b- Tuỳ theo mục đích, qui mô của kho (kho của nhà sản xuất, kho của nhà phân phối, kho của khoa dược bệnh viện...) cần phải có những khu vực xác định, được xây dựng, bố trí hợp lý, trang bị phù hợp:
+ Khu vực tiếp nhận, biệt trữ và bảo quản thuốc, nguyên liệu chờ nhập kho.
+ Khu vực lấy mẫu thuốc, nguyên liệu: khu vực này phải được xây dựng, trang bị thích hợp và phải có hệ thống cung cấp không khí sạch đảm bảo yêu cầu của việc lấy mẫu.
+ Khu vực bảo quản thuốc.
+ Khu vực bảo quản thuốc, nguyên liệu có yêu cầu bảo quản đặc biệt;
+ Khu vực bảo quản thuốc, nguyên liệu không đạt tiêu chuẩn chất lượng, chờ xử lý;
+ Khu vực bảo quản nguyên liệu, thành phẩm đã xuất kho chờ đưa vào sản xuất hoặc cấp phát;
+ Khu vực đóng gói, ra lẻ và dán nhãn;
+ Khu vực bảo quản bao bì đóng gói;
+ Khu vực bảo quản biệt trữ trước khi xuất nguyên vật liệu;
c. Nhà kho phải được thiết kế, xây dựng, bố trí đáp ứng các yêu cầu về đường đi lại, đường thoát hiểm, hệ thống trang bị phòng cháy, chữa cháy.
d- Trần, tường, mái nhà kho phải được thiết kế, xây dựng đảm bảo sự thông thoáng, luân chuyển của không khí, vững bền chống lại các ảnh hưởng của thời tiết như nắng, mưa, bão lụt.
đ- Nền kho phải đủ cao, phẳng, nhẵn, đủ chắc, cứng và được xử lý thích hợp để chống ẩm, chống thấm đảm bảo hoạt động của nhân viên làm việc trong kho, và hoạt động của các phương tiện cơ giới. Nền kho không được có các khe, vết nứt gãy .. là nơi tích luỹ bụi, trú ẩn của sâu bọ, côn trùng.
2.3 Trang thiết bị:
Nhà kho phải đáp ứng các yêu cầu sau:
a- Có các phương tiện, thiết bị phù hợp: quạt thông gió, hệ thống điều hòa không khí, xe chở hàng, xe nâng, nhiệt kế, ẩm kế... để đảm bảo các điều kiện bảo quản.
b- Có đủ ánh sáng bảo đảm để các hoạt động trong khu vực kho được chính xác và an toàn.
c- Có đủ các trang bị, giá, kệ để sắp xếp hàng hoá. Không được để thuốc, nguyên liệu trực tiếp trên nền kho. Khoảng cách giữa các giá kệ, giữa giá kệ với nền kho phải đủ rộng đảm bảo cho việc vệ sinh kho, kiểm tra, đối chiếu, cấp phát và xếp, dỡ hàng hóa.
d- Có đủ các trang thiết bị, các bản hướng dẫn cần thiết cho công tác phòng chống cháy nổ, như : hệ thống báo cháy tự động, thùng cát, hệ thống nước và vòi nước chữa cháy, các bình khí chữa cháy, hệ thống phòng chữa cháy tự động...
đ- Có nội quy qui định việc ra vào khu vực kho, và phải có các biện pháp phòng ngừa, ngăn chặn việc ra vào của người không được phép.
e- Có các qui định và biện pháp để chống sự xâm nhập, phát triển của côn trùng, sâu bọ, loài gặm nhấm...
2.4 Các điều kiện bảo quản trong kho:
Về nguyên tắc các điều kiện bảo quản phải là điều kiện ghi trên nhãn thuốc. Theo qui định của Tổ chức Y tế thế giới, điều kiện bảo quản bình thường là bảo quản trong điều kiện khô, thoáng, và nhiệt độ từ 15-250C hoặc tuỳ thuộc vào điều kiện khí hậu, nhiệt độ có thể lên đến 300C. Phải tránh ánh sáng trực tiếp gay gắt, mùi từ bên ngoài vào và các dấu hiệu ô nhiễm khác.
Nếu trên nhãn không ghi rõ điều kiện bảo quản, thì bảo quản ở điều kiện bình thường. Trường hợp ghi là bảo quản ở nơi mát, đông lạnh .... thì vận dụng các qui định sau:
a- Nhiệt độ:
Kho nhiệt độ phòng: Nhiệt độ trong khoảng 15-250C, trong từng khoảng thời gian nhiệt độ có thể lên đến 300C.
Kho mát: Nhiệt độ trong khoảng 8-150C.
Kho lạnh: Nhiệt độ không vượt quá 80C. Tủ lạnh: Nhiệt độ trong khoảng 2-80C.
Kho đông lạnh: Nhiệt độ không vượt quá - 100C.
b- Độ ẩm : Điều kiện bảo quản "khô" được hiểu là độ ẩm tương đối không quá 70%.
2.5 Kho bảo quản thuốc, nguyên liệu có yêu cầu bảo quản đặc biệt:
a- Các biện pháp đặc biệt cần được thực hiện đối với việc bảo quản các chất độc, chất nhạy cảm với ánh sáng, nhiệt độ, độ ẩm..., chất có hoạt tính cao, và chất nguy hiểm, như: các chất lỏng, chất rắn cháy nổ, các khí nén, các thuốc gây nghiện và các chất tương tự, các thuốc và hoá chất có độc tính cao, các vật liệu phóng xạ, dược liệu.
b- Các thuốc, nguyên liệu đòi hỏi các điều kiện bảo quản đặc biệt, cần phải được bảo quản ở các khu vực riêng biệt được xây dựng và trang bị thích hợp để đảm bảo các điều kiện bảo quản theo yêu cầu và các qui định của pháp luật.
c- Đối với các chất lỏng, rắn dễ cháy nổ, các khí nén... phải được bảo quản trong kho được thiết kế, xây dựng thích hợp cho việc bảo quản các sản phẩm cháy nổ theo qui định của pháp luật, phải xa các kho khác và xa khu vực nhà ở. Kho phải thông thoáng và được trang bị đèn chống cháy nổ. Các công tắc điện phải được đặt ngoài kho.
d- Đối với thuốc độc, thuốc gây nghiện, thuốc hướng tâm thần: phải được bảo quản tại khu vực kho đáp ứng qui định tại các qui chế liên quan.
đ- Các thuốc, hoá chất có mùi như tinh dầu các loại, amoniac, cồn thuốc ... cần được bảo quản trong bao bì kín, tại khu vực riêng, tránh để mùi hấp thụ vào các thuốc khác.
e- Đối với thuốc đòi hỏi điều kiện bảo quản có kiểm soát về nhiệt độ, độ ẩm, ánh sáng thì những điều kiện này phải được theo dõi và bảo đảm duy trì liên tục.
Các thiết bị được sử dụng để theo dõi điều kiện bảo quản: nhiệt kế, ẩm kế... phải được kiểm tra định kỳ, hiệu chỉnh khi cần, và kết quả kiểm tra, hiệu chỉnh này phải được ghi lại và lưu trữ.
g- Khu vực lấy mẫu, hoặc cấp phát lẻ các nguyên liệu, sản phẩm chờ đóng gói cần phải tách biệt khỏi các khu vực bảo quản khác, và phải được trang bị, có các dụngcụ cần thiết cho tiến hành công việc, phải có đủ các thiết bị cung cấp và thải khí, phòng chống tạp nhiễm, nhiễm chéo.
h- Phải thực hiện các biện pháp thích hợp để phòng ngừa sự tạp nhiễm, nhiễm chéo và cung cấp các điều kiện làm việc an toàn cho công nhân.
3.1 Khu vực bảo quản phải sạch, không có bụi rác tích tụ và không được có côn trùng sâu bọ. Phải có văn bản qui định chương trình vệ sinh, xác định rõ tần số và phương pháp vệ sinh nhà xưởng, kho.
3.2 Tất cả thủ kho, công nhân làm việc tại khu vực kho phải được kiểm tra sức khoẻ định kỳ. Người mắc các bệnh về đường hô hấp, hoặc có vết thương hở đều không được làm việc trong khu vực bảo quản có trực tiếp xử lý thuốc (nguyên liệu, thành phẩm...) còn hở.
Nơi rửa tay, phòng vệ sinh phải được thông gió tốt và bố trí phù hợp (cách ly với khu vực tiếp nhận, bảo quản, xử lý thuốc).
3.3 Công nhân làm việc trong khu vực kho phải mặc quần áo bảo hộ lao động thích hợp.
4.1 Yêu cầu chung :
a- Thuốc, nguyên liệu cần được bảo quản trong các điều kiện đảm bảo được chất lượng của chúng. Thuốc, nguyên liệu cần được luân chuyển để cho những lô nhận trước hoặc có hạn dùng trước sẽ đem sử dụng trước. Nguyên tắc nhập trước - xuất trước (FIFO- First In /First Out) hoặc hết hạn trước - xuất trước (FEFO- First Expired/ First Out) cần phải được thực hiện.
b- Thuốc chờ loại bỏ cần phải có nhãn rõ ràng và được biệt trữ nhằm ngăn ngừa việc đưa vào sản xuất, lưu thông, sử dụng.
c- Tuỳ theo tính chất và điều kiện bảo quản của sản phẩm, phải qui định chương trình kiểm tra, đánh giá định kỳ hoặc đột xuất để xác định chất lượng sản phẩm.
d- Phải có hệ thống sổ sách, các qui trình thao tác chuẩn đảm bảo cho
công tác bảo quản, kiểm soát, theo dõi việc xuất, nhập và chất lượng thuốc.
4.2 Nhãn và bao bì.
a- Các thuốc phải được bảo quản trong bao bì thích hợp, không ảnh hưởng xấu đến chất lượng của thuốc, đồng thời có khả năng bảo vệ thuốc khỏi các ảnh hưởng của môi trường, trong một số trường hợp, khi có yêu cầu, điều này bao gồm cả việc chống nhiễm khuẩn.
b- Trên tất cả các bao bì của thuốc phải có nhãn rõ ràng, dễ đọc, có đủ các nội dung, hình thức đáp ứng các qui định của pháp luật về nhãn và nhãn hiệu hàng hoá của thuốc. Không được sử dụng tên thuốc viết tắt, tên hoặc mã số không được phép.
c- Đối với mỗi loại bao bì, nhãn hoặc sản phẩm, phải có các hồ sơ ghi chép riêng biệt, trong đó chỉ ra các điều kiện bảo quản, các biện pháp đề phòng cần được chú ý và hạn dùng (nếu có).
d- Phải có khu vực riêng để bảo quản nhãn thuốc và các bao bì đóng gói đã được in ấn. Phải có qui định cụ thể cho việc nhập, cấp phát các loại nhãn và bao bì này.
đ- Phải tuân thủ các yêu cầu của dược điển và các qui định pháp luật liên quan đến nhãn và bao bì.
4.3 Tiếp nhận thuốc.
a- Việc tiếp nhận thuốc phải được thực hiện tại khu vực dành riêng cho việc tiếp nhận thuốc, tách khỏi khu vực bảo quản. Khu vực này phải có các điều kiện bảo quản để bảo vệ thuốc tránh khỏi các ảnh hưởng xấu của thời tiết trong suốt thời gian chờ bốc dỡ, kiểm tra thuốc.
b- Thuốc trước khi nhập kho phải được kiểm tra, đối chiếu với các tài liệu chứng từ liên quan về chủng loại, số lượng, và các thông tin khác ghi trên nhãn như tên hàng , nhà sản xuất, nhà cung cấp, số lô , hạn dùng...
c- Các lô hàng phải được kiểm tra về độ đồng nhất, và nếu cần thiết, được chia thành các lô nhỏ theo số lô của nhà cung cấp.
d- Tất cả các bao bì đóng gói cần được kiểm tra cẩn thận về độ nhiễm bẩn và mức độ hư hại, và nếu cần thiết, cần được làm sạch hoặc để riêng những bao bì nhiễm bẩn, bị hư hại để xem xét tìm nguyên nhân. Tất cả các thuốc có bao bì bị hư hại, mất dấu niêm phong hoặc bị nghi ngờ có tạp nhiễm thì phải được bảo quản ở khu vực biệt trữ để chờ xử lý, không được bán, hoặc để lẫn với các thuốc khác.
đ- Các thuốc đòi hỏi điều kiện bảo quản đặc biệt (các thuốc gây nghiện, thuốc độc, các thuốc cần bảo quản ở nhiệt độ lạnh...) phải nhanh chóng được kiểm tra, phân loại và bảo quản theo các chỉ dẫn ghi trên nhãn và theo các qui định của pháp luật.
e- Phải có và lưu các hồ sơ ghi chép cho từng lần nhập hàng, với từng lô hàng. Các hồ sơ này phải thể hiện được tên thuốc, dạng thuốc, nồng độ hàm lượng, chất lượng, số lượng, số lô sản xuất, hạn dùng, thời gian nhận hàng, và mã số (nếu có). Cần phải tuân thủ các qui định của pháp luật về lưu trữ hồ sơ.
g- Việc lấy mẫu thuốc để kiểm tra chất lượng phải được tiến hành tại khu vực dành cho việc lấy mẫu, và do người có trình độ chuyên môn thực hiện. Việc lấy mẫu phải theo đúng qui định tại Qui chế lấy mẫu thuốc để xác định chất lượng.
Lô thuốc, nguyên liệu đã được lấy mẫu phải được bảo quản biệt trữ. Các lô thuốc phải được để riêng biệt trong thời gian biệt trữ và trong thời gian bảo quản tiếp theo.
h- Chế độ biệt trữ phải được thực hiện hoặc bằng việc sử dụng khu bảo quản riêng biệt, hoặc bằng hệ thống xử lý dữ liệu điện tử.
Các biện pháp được áp dụng cần phải đủ độ an toàn để phòng tránh việc sử dụng hoặc cấp phát thuốc, nguyên liệu chưa được kiểm soát, kiểm nghiệm hoặc không đáp ứng yêu cầu qui định.
i- Thuốc, nguyên liệu cần phải được lưu giữ trong chế độ biệt trữ cho đến khi có văn bản chấp nhận hoặc loại bỏ của phòng kiểm tra chất lượng. Các biện pháp an ninh cần phải được thực hiện để đảm bảo thuốc, nguyên liệu bị loại bỏ sẽ không được sử dụng. Trong khi chờ quyết định huỷ, tái xử lý hoặc trả lại nhà cung cấp, các thuốc, nguyên liệu này phải được bảo quản riêng biệt với các thuốc, nguyên liệu khác
4.4 Cấp phát - quay vòng kho.
a- Chỉ được cấp phát các thuốc, nguyên liệu đạt tiêu chuẩn chất lượng, còn trong hạn sử dụng. Không được cấp phát, phân phối các thuốc, nguyên liệu không còn nguyên vẹn bao bì, hoặc có nghi ngờ về chất lượng.
b- Phải có và lưu các bản ghi chép (phiếu theo dõi xuất-nhập thuốc, phiếu theo dõi chất lượng thuốc...) thể hiện tất cả các lần nhập kho, xuất kho của thuốc, nguyên liệu phù hợp với số lô sản xuất.
c- Việc cấp phát cần phải tuân theo các nguyên tắc quay vòng kho (nhập trước-xuất trước hoặc hết hạn trước - xuất trước), đặc biệt là thuốc, nguyên liệu có hạn dùng. Chú ý khi một loại thuốc, nguyên liệu nhập sau có hạn dùng ngắn hơn thuốc, nguyên liệu cùng loại được nhập trước đó thì thuốc, nguyên liệu có hạn dùng ngắn hơn phải được xuất, cấp phát trước.
d- Các thùng, bao thuốc, nguyên liệu đã được sử dụng một phần cần phải được đóng kín lại một cách an toàn để tránh việc rơi vãi hoặc tạp nhiễm, nhiễm chéo trong thời gian bảo quản .
e- Các thùng, bao thuốc nguyên liệu bị hư hỏng, không còn nguyên niêm phong, mất nhãn hoặc nhãn bị rách, không rõ ràng thì không được bán, cấp phát, và phải thông báo ngay với bộ phận kiểm tra chất lượng.
4.5 Bảo quản thuốc.
a- Các điều kiện bảo quản được yêu cầu như: chủng loại bao bì, giới hạn nhiệt độ, độ ẩm, việc bảo vệ tránh ánh sáng... cần được duy trì trong suốt thời gian bảo quản. Cần phải có sự chú ý tới các thuốc chứa hoạt chất kém vững bền đối với nhiệt độ, độ ẩm, ánh sáng... (Tham khảo Phụ lục -Danh mục một số hoạt chất ít vững bền)
b- Bao bì thuốc phải được giữ nguyên vẹn trong suốt quá trình bảo quản. Không dùng lẫn lộn bao bì đóng gói của loại này cho loại khác.
c- Thuốc độc, thuốc gây nghiện, thuốc hướng tâm thần phải được bảo quản theo đúng các qui định tại qui chế liên quan.
Các thuốc nhạy cảm với nhiệt độ phải được bảo quản ở kho lạnh hoặc trong tủ lạnh. Nhiệt độ trong kho phải được kiểm tra ở các vị trí khác nhau của kho.
Các thuốc nhạy cảm với ánh sáng phải được bảo quản trong bao bì kín, không cho ánh sáng truyền qua, trong phòng tối.
Các thuốc dễ bay hơi và các thuốc nhạy cảm với độ ẩm phải được bảo quản tại kho lạnh, bao bì đóng kín. Các chất hút ẩm mạnh phải được bảo quản tại phòng khô, bao bì bằng thuỷ tinh hoặc nhựa đóng kín. Nếu có thể thì nút phải được phủ paraffin.
Các thuốc có mùi phải được bảo quản trong bao bì kín, ở kho riêng.
Dược liệu phải được bảo quản ở kho khô, thông thoáng. Các thùng hàng phải được sắp xếp hợp lý đảm bảo cho không khí lưu thông. Các vật liệu thích hợp để làm bao bì bảo quản dược liệu có thể là thuỷ tinh, nhựa, giấy... Các dược liệu chứa tinh dầu cũng cần phải được bảo quản trong bao bì kín.
Các chất dễ cháy, nổ phải được bảo quản tại kho riêng, đáp ứng các qui định của pháp luật.
d- Phải định kỳ tiến hành việc đối chiếu thuốc trong kho theo cách so sánh thuốc hiện còn và lượng hàng còn tồn theo phiếu theo dõi xuất nhập thuốc. Trong mọi trường hợp, việc đối chiếu phải được tiến hành khi mỗi lô hàng được sử dụng hết.
đ- Tất cả các sai lệch, thất thoát cần phải được điều tra để tìm ra nguyên nhân do lẫn lộn, cẩu thả hay các vấn đề sai trái khác.
e- Thường xuyên kiểm tra số lô và hạn dùng để đảm bảo nguyên tắc nhập trước - xuất trước hoặc hết hạn trước- xuất trước được tuân thủ, và để phát hiện hàng gần hết hoặc hết hạn dùng.
g- Định kỳ kiểm tra chất lượng của hàng lưu kho để phát hiện các biến chất, hư hỏng trong quá trình bảo quản do điều kiện nhiệt độ, độ ẩm hoặc các yếu tố khác có thể ảnh hưởng đến chất lượng thuốc, nguyên liệu.
h- Thuốc, nguyên liệu không đạt tiêu chuẩn chất lượng, hết hạn dùng phải được bảo quản ở khu vực riêng, phải dán nhãn, có biển hiệu thuốc,nguyên liệu chờ xử lý. Phải có các biện pháp đề phòng việc cấp phát, sử dụng thuốc, nguyên liệu đã hết hạn dùng, thuốc, nguyên liệu không đạt tiêu chuẩn chất lượng.
i- Phải có các phương tiện vận chuyển và bảo quản thích hợp nhằm đảm bảo cho thuốc, nguyên liệu tránh đổ vỡ và hư hỏng do các điều kiện khí hậu vượt quá qui định như nắng nóng, ẩm ướt... trong suốt quá trình vận chuyển, bảo quản cần lưu ý những loại thuốc, nguyên liệu có yêu cầu điều kiện bảo quản đặc biệt.
5.1 Tất cả các thuốc đã xuất ra khỏi kho, bị trả về phải được bảo quản tại khu biệt trữ. Các thuốc này chỉ được đưa trở lại kho thuốc để lưu thông, phân phối, sử dụng sau khi bộ phận bảo đảm chất lượng đánh giá là thuốc đạt tiêu chuẩn chất lượng và đảm bảo an toàn cho người sử dụng.
5.2 Tất cả các thuốc trả về, sau khi được bộ phận bảo đảm chất lượng đánh giá là không đảm bảo chất lượng, không đảm bảo an toàn cho người sử dụng thì không được đưa vào sử dụng và phải được xử lý theo qui định của pháp luật.
5.3 Những thuốc do bệnh nhân trả lại phải được để ở khu vực riêng, chờ huỷ bỏ.
6. Gửi hàng (Vận chuyển thuốc bằng cách gửi hàng)
6.1 Việc cấp phát và xếp hàng lên phương tiện vận chuyển chỉ được thực hiện sau khi có lệnh xuất hàng bằng văn bản.
Các nguyên tắc, qui định về qui trình vận chuyển thuốc bằng cách gửi hàng (dispatch) phải được thiết lập tuỳ theo bản chất của thuốc và sau khi đã cân nhắc tất cả các biện pháp phòng ngừa.
6.2 Đối với những thuốc có yêu cầu bảo quản đặc biệt, trong thời gian vận chuyển, phải đảm bảo duy trì các điều kiện đó.
6.3 Việc vận chuyển các thuốc độc, thuốc gây nghiện, thuốc hướng tâm thần phải tuân thủ theo đúng qui định tại các qui chế liên quan.
6.4 Các bao bì vận chuyển cần phải bảo vệ thuốc tránh các ảnh hưởng bên ngoài và phải được dán nhãn rõ ràng, không bị tẩy xoá hoặc dễ bị tẩy xoá.
6.5 Tài liệu vận chuyển gửi hàng cần phải ghi rõ :
- Thời gian vận chuyển.
- Tên và địa chỉ khách hàng.
- Tên và địa chỉ người gửi.
- Tên và địa chỉ người vận chuyển.
- Tên thuốc, dạng thuốc, hàm lượng, số lượng và số lô.
- Điều kiện vận chuyển, bảo quản.
6.6 Tất cả tài liệu liên quan đến việc vận chuyển, gửi hàng phải được lưu tại bên gửi và bên nhận hàng, và được bảo quản tại nơi an toàn.
7.1 Qui trình thao tác: cần phải có sẵn, treo tại các nơi dễ đọc các qui trình thao tác chuẩn đã được phê duyệt xác định phương pháp làm việc trong khu vực nhà kho. Các qui trình này phải mô tả chính xác các qui trình về tiếp nhận và kiểm tra thuốc nhập kho, bảo quản, vệ sinh và bảo trì kho tàng, thiết bị dùng trong bảo quản (bao gồm cả các qui trình kiểm tra, kiểm soát côn trùng, chuột bọ..), qui định về việc ghi chép các điều kiện bảo quản, an toàn thuốc tại kho và trong quá trình vận chuyển, việc cấp phát thuốc, các bản ghi chép, bao gồm cả các bản ghi về đơn đặt hàng của khách hàng, thuốc trả về, qui trình thu hồi và xác định đường đi của thuốc, và của thông tin .... Các qui trình này phải được xét duyệt, ký xác nhận và ghi ngày tháng xét duyệt bởi người có thẩm quyền.
7.2 Phải có một hệ thống sổ sách thích hợp cho việc ghi chép, theo dõi việc xuất nhập các thuốc, bao gồm tên thuốc, số lô, hạn dùng, số lượng, chất lượng thuốc, nhà cung cấp, nhà sản xuất... đáp ứng các qui định của pháp luật. Nếu các loại sổ sách được vi tính hoá thì phải tuân theo các qui định của pháp luật. Phải có các qui định, biện pháp phòng ngừa cụ thể để tránh việc xâm nhập, sử dụng, sửa chữa một cách bất hợp pháp các số liệu được lưu giữ.
- Phiếu theo dõi xuất nhập thuốc (mẫu đính kèm số 1-GSP/MB)
- Phiếu theo dõi chất lượng thuốc (mẫu đính kèm số 2-GSP/MB)
- Các biểu mẫu khác theo qui định của các Bộ Ngành có liên quan.
Phải có phiếu theo dõi xuất nhập thuốc riêng cho từng loại sản phẩm cũng như cho từng loại qui cách sản phẩm.
Đối với việc cấp phát, tiếp nhận thuốc độc, thuốc gây nghiện, thuốc hướng tâm thần phải tuân theo đúng các qui định về hồ sơ tài liệu tại các qui chế liên quan.
1.1. Các đơn vị xuất nhập khẩu, buôn bán, tồn trữ thuốc, các đơn vị làm dịch vụ kho bảo quản thuốc, khoa dược bệnh viện, viện nghiên cứu, trung tâm y tế phải xây dựng kế hoạch từng bước đầu tư nâng cấp, xây dựng kho thuốc theo nguyên tắc “ Thực hành tốt bảo quản thuốc”.
1.2. Các đơn vị sản xuất thuốc trong quá trình triển khai áp dụng các nguyên tắc tiêu chuẩn “Thực hành tốt sản xuất thuốc” đồng thời phải triển khai áp dụng các nguyên tắc “Thực hành tốt bảo quản thuốc”.
2.1 Đào tạo
a- Bộ Y tế (Cục Quản lý dược Việt Nam) tổ chức phổ biến, huấn luyện về
“Thực hành tốt bảo quản thuốc” cho Tổng Công ty dược Việt Nam, Sở Y tế các tỉnh, thành phố trực thuộc Trung ương, Y tế các ngành, các đơn vị sản xuất, kinh doanh, các bệnh viện trung ương và các viện có giường bệnh.
b- Sở Y tế các tỉnh, thành phố trực thuộc Trung ương, Tổng Công ty dược Việt Nam, Y tế các ngành có trách nhiệm phổ biến, huấn luyện, và đào tạo các nguyên tắc “Thực hành tốt bảo quản thuốc” cho các đơn vị trực thuộc, kể cả khoa dược bệnh viện, viện nghiên cứu và trung tâm y tế đóng trên địa bàn.
c- Các đơn vị tự phổ biến huấn luyện các nguyên tắc "Thực hành tốt bảo quản thuốc" cho cán bộ công nhân viên của đơn vị.
2.2 Thủ tục đăng ký và thẩm quyền kiểm tra.
2.2.1 Đăng ký kiểm tra
Các đơn vị, sau khi tự kiểm tra, đánh giá cơ sở mình đạt tiêu chuẩn “Thực hành tốt bảo quản thuốc”, gửi Hồ sơ đăng ký kiểm tra “Thực hành tốt bảo quản thuốc” đến Bộ Y tế (Cục Quản lý dược Việt Nam). Hồ sơ đăng ký gồm:
- Bản đăng ký kiểm tra “Thực hành tốt bảo quản thuốc” (mẫu đính kèm số 3-GSP/MB)
- Chương trình, nội dung và kết quả huấn luyện“Thực hành tốt bảo quản thuốc” tại cơ sở.
- Sơ đồ tổ chức, biên chế của cơ sở.
- Sơ đồ vị trí địa lý và thiết kế của kho.
- Giấy xác nhận đảm bảo môi trường của cơ quan có thẩm quyền.
- Danh mục trang thiết bị bảo quản của cơ sở.
- Danh mục chủng loại mặt hàng bảo quản tại kho.
- Biên bản tự thanh tra “ Thực hành tốt bảo quản thuốc”
Hồ sơ đăng ký được làm thành 03 bộ, gửi về Cục Quản lý dược Việt Nam.
2.2.2 Hồ sơ đăng ký tái kiểm tra:
Hồ sơ đăng ký tái kiểm tra bao gồm:
- Bản đăng ký tái kiểm tra “Thực hành tốt bảo quản thuốc”
- Báo cáo những thay đổi về biên chế, tổ chức, nhà xưởng của cơ sở trong 02 năm triển khai “Thực hành tốt bảo quản thuốc”.
- Báo cáo tóm tắt về chương trình, nội dung và kết quả huấn luyện“Thực hành tốt bảo quản thuốc” tại cơ sở trong 02 năm.
- Báo cáo tự thanh tra, và đánh giá của cơ sở trong đợt gần nhất về triển khai “Thực hành tốt bảo quản thuốc”.
2.2.3 Thẩm quyền kiểm tra và cấp chứng nhận:
Cục Quản lý dược Việt Nam chịu trách nhiệm tiếp nhận, thẩm định hồ sơ đăng ký kiểm tra, thành lập đoàn kiểm tra cơ sở và cấp chứng nhận cơ sở đạt tiêu chuẩn “Thực hành tốt bảo quản thuốc”.
2.2.3.1 Tiêu chuẩn của cán bộ kiểm tra Thực hành tốt bảo quản thuốc :
- Có trình độ đại học trở lên, có kinh nghiệm trong công tác quản lý dược nói chung và bảo quản thuốc nói riêng.
- Trung thực khách quan.
- Có phương pháp làm việc khoa học, cương quyết nghiêm chỉnh chấp hành các qui chế, qui định, có khả năng phát hiện nhanh các sai sót của cơ sở, đồng thời phải đưa ra được các biện pháp có tính thuyết phục giúp cơ sở khắc phục thiếu sót.
2.2.3.2 Thẩm định hồ sơ đăng ký kiểm tra Thực hành tốt bảo quản thuốc .
Sau khi nhận đủ hồ sơ đăng ký kiểm tra “Thực hành tốt bảo quản thuốc” của cơ sở theo qui định (tiết 2.2.1 và 2.2.2), Cục Quản lý dược Việt Nam kết hợp với chuyên gia của các cơ quan liên quan thẩm định hồ sơ (có biên bản thẩm định hồ sơ). Trong vòng 30 ngày kể từ ngày nhận đủ hồ sơ hợp lệ, Cục Quản lý dược Việt Nam có quyết định thành lập đoàn kiểm tra đến kiểm tra “Thực hành tốt bảo quản thuốc” tại cơ sở. Nếu hồ sơ chưa đáp ứng đầy đủ các yêu cầu, Cục Quản lý dược Việt Nam phải có công văn trả lời cơ sở và nêu rõ lý do.
2.2.3.3 Kiểm tra và cấp giấy chứng nhận:
- Thành phần đoàn kiểm tra: Cục Quản lý dược Việt Nam và Sở Y tế tỉnh, thành phố trực thuộc Trung ương nơi doanh nghiệp đóng trụ sở.
- Biên bản kiểm tra phải được phụ trách cơ sở và trưởng đoàn kiểm tra ký xác nhận, được làm thành 03 bản: 01 bản lưu tại cơ sở, 01 bản lưu tại Cục Quản lý dược Việt Nam và 01 bản lưu tại Sở Y tế tỉnh, thành phố trực thuộc Trung ương nơi đơn vị đóng trụ sở.
Sau khi kiểm tra, nếu cơ sở đáp ứng được các nguyên tắc “Thực hành tốt bảo quản thuốc”, Bộ Y tế (Cục Quản lý dược Việt Nam) sẽ cấp giấy chứng nhận đạt tiêu chuẩn”Thực hành tốt bảo quản thuốc”. Giấy chứng nhận “Thực hành tốt bảo quản thuốc” có giá trị 02 năm kể từ ngày ký. Trước khi hết hạn 03 tháng, cơ sở phải làm thủ tục đăng ký tái kiểm tra theo qui định trong tiết 2.2.2, điểm 2.2, mục 2, phần III của bản nguyên tắc này. Nếu đạt tiêu chuẩn theo qui định, cơ sở sẽ được tiếp tục cấp giấy chứng nhận mới.
Định kỳ kiểm tra “Thực hành tốt bảo quản thuốc” 02 năm 01 lần, trừ các trường hợp đột xuất do cơ sở hoặc Bộ Y tế (Cục Quản lý dược) yêu cầu.

DECISION
APPLICATION OF “GOOD STORAGE PRACTICES” PRINCIPLE
THE MINISTER OF HEALTH
Pursuant to the Law on People’s Health Protection dated July 11, 1989 and Statute of preventive and treatment drug enclosed together with Decree No. 23/HDBT dated January 24, 1991 of the Council of Ministers (now the Government);
Pursuant to the Ordinance on goods quality dated December 24, 1999;
Pursuant to the Decree No. 68/CP dated October 11, 1993 by the Government stipulating functions, duties, authorities and organizational structures of the Ministry of Health;
Pursuant to the Decree No. 86/CP dated December 08, 1995 of the Government stipulating assignment of state management responsibilities for goods quality;
At the proposal of General Director of Drug administration of Vietnam,
DECISION
Article 1. Developing application of “Good storage practices” principle enclosed together with this Decision to pharmaceutical manufacturers, importers, exporters, wholesalers, contractors, drug storage and conservation service companies, hospital’s pharmacy departments, Research Institute and medical centers.
Article 2. This Decision is effective after 15 days since the date of signing and promulgating.
Article 3. Mr. General Director of Drug administration of Vietnam shall be responsible for instructing, inspecting and monitoring the application of the principle “Good drug protecting practice”.
Article 4. The Chief Officers, the Chief Inspector of the Ministry of Health, General Director of Drug administration of Vietnam, heads of agencies under the Ministry, directors of provincial and municipal departments of health under the Center, head physicians of various branches are responsible for executing this Decision.

“GOOD STORAGE PRACTICE”

Part 1.
GENERAL PART
1. Purposes.
To ensure supply of quality drug to consumers, it is required that all the stages ranging from production, conservation, storage, circulation and distribution of drug are well implemented.
Good storage practices are special and suitable measures for storage and delivery of materials and products from production, conservation, storage, delivery and distribution to ensure the drug attain intended quality when reaching consumers.
This document stipulates basic principles and general instructions on “Good storage practices”. However, these principles, guidances can be modified to meet particular requirements but still ensure intended quality.
The principle “Good storage practices" applies to pharmaceutical manufacturers, importers, exporters, wholesalers, contractors, drug storage and conservation service companies, hospital’s pharmacy departments, Research Institute and medical centers.
2. Interpretation of terms
Terms used in this Principle are construed as follows:
2.1 Drug storage means safe storage of drugs, materials including putting into use and sufficiently maintaining appropriate systems of documents and materials including receipts and delivery orders.
2.2 Drugs means products that originate from animals, plants, minerals, pharmaceutical chemistry or biology and processed for human use, in order to:
- Prevent and treat diseases
- Restore and adjust physical functions of human bodies
- Reduce symptoms of diseases
- Diagnose diseases
- Help recover or improve health
- Desensitize part or whole of the body.
- Influence reproduction processes.
- Change body shapes
2.3 Materials mean substances active or inactive, changeable or unchangeable used in manufacture of drugs. They include active ingredients, excipients, reagents, solvents, intermediary products, packages and labels.
2.4 Active ingredient means pharmaceutically active substances or compounds used in manufacture of drugs.
2.5 Excipient means substances, other than active ingredients, adequately assessed of safety and put into drug distribution system.
2.6 Packages mean every material used for packaging products except containers used to store products and transported by various means of transport to recipients.
2.7 Intermediate product means a partly processed material which must undergo further processing before becoming a finished product.
2.8 Finished product means medicinal products having undergone all stages of production including packaging.
2.9 Retest date means The date when drug, materials should be re-examined to ensure that it is still suitable for use.
2.10 Labels mean printouts, pictures, images, signs being engraved, embossed or attached directly to commercial packages to convey crucial information of that drug, helping users to choose and use right drug and functioning as the basis for functional agencies to carry out inspection, supervision and management.
Labels include all labels, texts or images printed, written on intermediary packages or on packages or wrappers of that product except containers.
2.11 Isolation means the status of materials or products set apart physically or by administrative system pending rejection or release for processing, packaging or distribution.
1.12 FIFO means an acronym for “First In/First Out”.
1.12 FIFO means an acronym for “First Expired/First Out”.
2.13 Contamination means unexpected appearance of impurities of chemical or microbiological nature, or foreign matters in the raw material, intermediate products, finished products during manufacture, sampling, packaging, re-packaging, storage and delivery.
2.14 Cross-contamination means the contamination of materials, intermediary products or finished products with other materials and products during manufacture, storage and delivery.
Part 2.
GOOD STORAGE PRACTICES
1. Staff:
1.1. According to the organization’s scale, storage site must be adequately staffed by those who are qualified for assigned tasks. Every staff must be regularly trained in “Good storage practices”, professional skills and clearly defined in writing in terms of responsibilities and duties of each person.
1.2. Storage site’s key officers functioning as supervisors, inspectors must be honest, have necessary knowledge, experience and professional and technical qualifications suitable for assigned tasks according to state regulations.
1.3. Store keepers must acquire necessary knowledge of pharmacy and drug storage such as method of storage, method of managing stock inventory and drug quality...
1.4. Store keepers must be certified at least an associate degree in pharmacy to work for modern medicines manufacturing and trading facilities. With respect to facilities manufacturing and trading traditional medicine, herbal ingredients, store keepers must be certified at least as pharmacy aides, pharmacy technicians.
1.5 Keepers of storage sites that have toxins, narcotics and psychotropic drugs must meet relevant regulations.
1.6. Store keepers must be regularly trained and updated with new regulations on drug storage and management, scientific and technical advances applied to drug storage.
2. Stores and facilities:
Stores must be designed, constructed, equipped, renovated and maintained systematically to facilitate protection of drugs and materials against possible adverse effects such as changes in temperature and humidity, waste and odor, animals, plants, and insects ensuring intended quality of drugs.
2.1 Location:
Stores must be constructed on a high, dry and safe place with a drainage system being installed to ensure drugs and materials are protected from effects of underground water, heavy rains, storms and flooding...
Every store must have a specific address, situated in a place favorable for reception, shipment and protection..
2.2 Design, construction:
a- Stores must be spacious enough and if necessary, separation can be established to ensure isolation of each type and batch of drugs on request.
b- Depending on purpose and scale of stores (manufacturer’s stores, distributor's stores, hospital’s pharmacy department’s stores...), some specific areas must be determined, constructed and equipped suitably.
+ Area for receiving, isolating and storing products, raw materials pending stocking.
+ Area for taking samples of products, materials: this area must be constructed and equipped suitably and have a clean air supply system to ensure requirements of sampling.
+ Area for storing drugs.
+ Area for storing drugs, raw materials of special storage requirements.
+ Area for receiving and storing drugs, raw materials failing to meet quality standards, awaiting handling;
+ Area for storing raw materials, finished products taken out for being put into production or dispensation;
+ Area for packaging, classification and labeling;
+ Area for storing packages;
+ Area for isolating materials before dispatch;
c. Stores must be designed, constructed and arranged in such a manner as to facilitate traveling, escape, fire prevention and control system.
d- Ceiling, wall, store roof must be designed, constructed to ensure ventilation, circulation of air, able to resist effects of sun, rain and storm.
dd- Storage areas must be high, flat, smooth, sufficiently firm and treated to withstand humidity and penetration, ensuring operation of store keepers and motor vehicles. Storage areas must not have fractures, slots... as places for accumulation of dirt and shelter of insects.
2.3 Facilities:
Stores must meet the following requirements:
a- Appropriate devices and equipment: blowers, air-conditioners, trucks, bucket truck, thermometer, hygrometer... are provided to ensure storage conditions.
b- Lighting in the warehouse must be adequate to enable all operations to be carried out accurately and safely.
c- Stores must provide adequate equipments, supports, shelves for arrangement of products. Products, materials are not allowed to be placed directly on the ground. Distances between the shelves and from the shelves to the floor must be enough to enable cleaning, inspecting, checking, dispensing and loading products.
d- Stores must provide adequate facilities and instructions necessary for fire prevention and fighting tasks such as automatic fire alarm system, sand box, fire hydrant and hose system, fire extinguishers, automatic fire prevention and control system...
dd- Regulations must be imposed to prevent unauthorized persons from entering storage areas
e- Regulations and measures must be taken to prevent intrusion and growth of insects and rodents...
2.4 Storage conditions:
Storage conditions, in principle, must be those that are required on labels. According to the WHO's regulations, normal storage conditions are storage in dry, well-ventilated premises at temperature of 15-250C or depending on climatic conditions, up to 300C. Extraneous odors, other indications of contamination, and intense light must be excluded.
If storage conditions are not clearly written on labels, normal storage conditions should be applied. In case storage in cool, freeze places is required on labels, the following instructions are recommended:
a--Temperature:
Controlled room temperature of 15-250C: depending on each period of time, temperature can reach up to 300C.
Cool storage temperature: from 8-150C
Cold storage temperature: Not exceeding 80C. Freezer temperature: from 2-80C
Dry ice: Not exceeding 100C.
b- Humidity: “dry” storage conditions are construed as relative humidity (not over 70%).
+ Area for storing drugs, materials of special storage requirements.
a- Special measures should be taken for the storage of toxins, materials sensitive to light, temperature, humidity..., highly active and dangerous materials such as combustible liquids, solids and pressurized gases, narcotics, highly toxic pharmaceutical products and chemicals, radioactive materials and herbal ingredients.
b- Pharmaceutical products, materials requiring special storage conditions shall be stored in an isolation area which is suitably constructed and equipped to ensure storage conditions according to requirements and regulations of the law.
c- For combustible solids, liquids, pressurized gases..., they should be stored in a store suitably designed and constructed to enable storage of combustible products according to the law and located far away from other stores and residential areas. Stores should provide adequate ventilation and flameproof lights. Electric switches should be positioned outside of stores.
d- For toxins, narcotics and psychotropic drugs, they should be stored in the areas meeting relevant regulations and statutes.
dd- For pharmaceutical products, chemicals with detectable smells such as essential oils, ammonia and tinctures..., they should be stored in closed packages in isolation areas to avoid mixing up with other drugs.
e- For pharmaceutical products requiring controlled storage conditions in temperature, humidity and light, these conditions should be monitored and guaranteed to be constantly maintained.
Equipment used to monitor storage conditions such as thermometers, hygrometer...must be periodically inspected, corrected when necessary, and the results from inspection, correction should be recorded and kept in archives.
g- Areas for sampling, or dispensing materials and products pending packaging should be separated from other storage areas and adequately equipped with necessary equipment for air supply and discharge, contamination and cross-contamination prevention.
h- Appropriate measures should be taken to prevent contamination, cross-contamination and provide safe working conditions for workers.
3. Sanitation:
3.1. Storage areas should be kept clean without accumulated dirt and insects. Documents stipulating sanitation program should be promulgated, frequency and method of cleaning stores and facilities be clearly defined.
3.2 All store keepers, workers working in the storage area should have regular check-ups. Any one suffering respiratory tract related diseases or open wounds shall not be allowed to work in the storage area where they work directly with drugs (materials, finished products...)Hand washing areas and restrooms must provide adequate ventilation and be appropriately arranged (isolated from areas of reception, storage and treatment of drugs).
3.3 Workers working in the storage area should wear appropriate protective clothing.
4. Storage procedures
4.1 General requirement:
a- Pharmaceutical products, materials should be stored in such conditions as to ensure their quality. Pharmaceutical products, materials should be rotated so that FIFO or FEFO principles are complied with.
b- Pharmaceutical products pending rejection should have clear labels and stored in isolation to prevent them from being put back into production, circulation and use.
c- Depending on characteristics of products and storage conditions, a regular or unexpected program of inspection and assessment should be planned to determine product quality.
d- A standardized system of records and operation procedures should be available to facilitate storage, control, monitoring of reception, dispatch and drug quality.
4.2 Labels and packages
a- Pharmaceutical products should be stored in appropriate packages in order not to adversely affect quality of products, and at the same time to be able to protect products against impact of the environment, and in certain circumstances, antibacterial factors if requested.
b- Labels attached to product packages must be clear, readable and written with adequate information and manners meeting regulations of the law on labels and trade marks of pharmaceutical products. Use of acronyms for drug names or codes is not permitted.
c- Each type of package, label or product should be accompanied by particular records describing storage conditions, precautions and expiry date (if any)
d- A separate area should be available to store product labels and printed packages. Specific regulations should be issued to facilitate reception, dispense of these labels and packages.
dd- Requirements of pharmacopoeia and regulations concerning labels and packages should be complied with.
4.3 Reception of products.
a- Reception of products should be carried out in an area intended for reception of products and isolated from storage area. This area should provide storage conditions for protecting products against adverse effects of weather during the process of loading and inspection of products.
b- Products before being received should be inspected and checked against documents, receipts concerning type, quantity and other information as described on the label such as product name, manufacturer, supplier, batch number and expiry date...
c- Batches should be inspected in its uniformity and if necessary, divided into small batches according to batch numbers of the supplier.
d- All packages should be carefully checked for contamination and damage. If necessary, contaminated or damaged can be cleaned or separated for determination of causes of such contamination or damage. All products contained in packages being damaged, lost of seal or showing signs of contamination should be stored in an isolation area pending treatment, and must not be sold or mixed up with other pharmaceutical products.
dd- Products requiring special storage conditions (narcotics, toxins and other materials requiring cold storage...) should be inspected quickly, classified and stored according to instructions described on the label and regulations of the law.
e- Archive of records for each reception of products and each batch should be available on request. These records should be fully described with such information as product name, dosage form, concentration, content, quality, quantity, manufacture batch, expiry date, date of reception and codes (if any). Regulations of the law on archive of records should be strictly complied with.
g- Sampling for quality inspection should be carried out in an area intended for sampling and undertaken by qualified staff. Task of sampling should meet regulations set out in the Statute on sampling of drugs for quality determination.
Batches, materials taken sample should be stored in isolation. Batches of products should be stored in isolation during a period of isolation and next storage period.
h- Isolation regime should be carried out either by use of special storage area or use of electronic data processing system.
Measures taken should ensure adequate safety to prevent use or dispense of products, materials which have not undergone testing, inspection or failing to meet requirements as regulated.
i- Products, materials should be stored in isolation until a written approval or rejection by quality inspection division is released. Security measures should be taken to ensure rejected products, materials are not put back into use. While awaiting a decision for rejection, re-processing or returning to supplier, these products, materials should be stored in isolation from other products, materials.
4.4 Dispense – stock rotation
a- Only products, materials being unexpired and meeting quality standards are dispensed. Dispense and delivery of products, materials of broken packages or uncertain quality is prohibited.
b- Records (dispatch and receipt notes; quality control sheets) on which the number of receipts and dispatches of products, materials is consistent with the number of batches.
c- Dispensing of products should comply with principles of stock rotation (first in – first out or first expired – first out), especially products, materials having limited life. When shelf-life of a successively received product, material of is shorter than that of the product, material of the same category previously received, those of a shorter shelf-life shall be dispatched and dispensed first.
d- Boxes, packages of once used products, materials should be safely sealed to avoid scattering or contamination, cross-contamination during the maintenance period.
e- Boxes, packages of products, materials being damaged, lost of seal, labels or with labels torn or unreadable shall not be permitted for sale, dispense and should be notified immediately to quality control department.
4.5 Storage of drugs
a- Such storage conditions as package types, temperature limit, humidity and lightproof protection....should be maintained during the period of storage. Attention should be paid to products containing active ingredients sensitive to temperature, humidity and light... (Refer to Appendix – List of some less persistent active ingredients)
b- Packages of products should be kept intact during storage. Do not confuse packages of this kind with others.
c- Toxins, narcotics and psychotropic drugs should be stored in accordance with regulations of the relevant Statute.
Pharmaceutical products sensitive to temperature should be stored in cold or frozen storage conditions. Storage temperature should be inspected at different positions in the store.
Pharmaceutical products sensitive to light should be stored in closed lightproof packages in dark area.
Pharmaceutical products volatile and sensitive to humidity should be stored in cold conditions, packages sealed. Strongly hygroscopic substances should be stored in glass or plastic packages closely sealed in a dry room. If possible, the stopper should be covered with paraffin.
Odorous products should be stored in sealed packages in a separate store.
Herbal ingredients should be stored in a dry and well-ventilated store. Boxes of products should be suitably arranged to enable circulation of air Materials appropriate for production of packages storing herbal ingredient can be glass, plastic and paper... Herbal ingredients containing essential oil should be stored in sealed packages.
Combustible and explosive substances should be stored in a separate dry store, meeting regulations of the law.
d- Regularly carry out checking inventory by comparing existing stock against the stocks described in dispatch and receiving follow-up vouchers. In all circumstances, the checking should be carried out when each batch is used up.
dd- All discrepancies and losses should be investigated to determine the cause due to errors, recklessness or other wrongdoings.
e- Inspection of batch number and shelf-life should be regularly carried out to ensure the principle “first in – first out”, or “first expired – first out” is complied with, and to detect whether products are sold out or expired.
d- Quality inspection of products in stock should be regularly carried out to detect degradation or deterioration during storage due to temperature and humidity conditions and other factors which may affect quality of products, materials.
h- Products, materials failing to meet quality standards or being expired should be stored in isolation, attached with labels, signboards for handling. h- Precautions should be taken to cope with dispense and use products, materials failing to meet quality standards or being expired.
i- Appropriate transportation and storage means should be available to ensure products, materials are not broken and damaged due to excessive climatic conditions according to the regulations such as hot sun and moisture...During the process of transportation and storage, attention should be paid to products, materials subject to requirements of special storage conditions.
5. Returned products
5.1 All products dispatched, returned should be stored in an isolation area. These products can be returned to the store for circulation, delivery and use only when they are evaluated by the quality assurance department as having met quality standards and ensured safety to users.
5.2 All returned products after being evaluated by the quality assurance department as failing to meet quality standards and safety to users shall not be put back into use and handled according to the law.
5.3 Products returned by patients should be stored in isolation pending rejection.
6. Dispatch
6.1 Dispensing and loading products onto transport vehicles are carried out only after a delivery order is released.
Principles, regulations on dispatch procedures should be established depending on the nature of products and after all precautions are put into consideration.
6.2 For products of special storage requirements, maintenance of regulated conditions should be ensured during transportation.
6.3 Transport of toxins, narcotics, psychotropic drugs should conform to the regulations as out in relevant Statute.
6.4 Outside containers should provide adequate protection from all external influences and should be indelibly and clearly labeled.
6.5 Records of dispatch should state:
- Date of dispatch
- Customer’s name and address
- Dispatcher’s name and address
- Transporter’s name and address
- Product name, dosage form, content, quantity and batch number.
- Transport and storage conditions.
6.6 All records of dispatch should be retained at both sides (dispatcher and customer) and available on request.
7. Document
7.1 Operation procedures: approved operation procedures determining working method in the store should be posted at places easily readable. These procedures should accurately describe the process of reception and inspection of received drugs, storage, sanitation and maintenance of treasure, equipment used in storage (including processes of inspection and control of insects, vermin...), regulations on recording storage and safety conditions in the store and during transportation, dispensing, records including records of place orders by customers, returned products, process of recall and determining the path of products and information. These procedures should be approved, countersigned with date of approval by competent persons.
7.2 Appropriate record system should be available for recording, monitoring reception and dispatch of products including product name, batch number, expiry date, quantity, quality, supplier and manufacturer....meeting regulations of the law. In case the record system is computerized, it is ensured that regulations of the law are complied with. Specific regulations, precautions should be available to avoid illegal intrusion, use and modification of stored data.

- Other forms according to regulations of relevant ministries and departments.
Reception and dispatch follow-up voucher should be made specifically for each type and specification of products.
Dispensing and receiving toxins, narcotics and psychotropic drugs should be in strict compliance with records related regulations as set out in relevant Statute.
Part 3.
GUIDANCE ON IMPLEMENTATION
1. General regulations:
1.1 Pharmaceutical manufacturers, importers, exporters, wholesalers, contractors, drug storage and conservation service companies, hospital’s pharmacy departments, Research Institute and medical centers should build a step-by-step plan for investment and construction of store according to "Good storage practices” principle.
1.2. While applying “Good manufacturing practices” principle, pharmaceutical manufacturers shall also apply “Good storage practices” principle.
2. Implementation
2.1 Training
a- The Ministry of Health (Drug administration of Vietnam) shall organize dissemination and training in “Good storage practices” to Vietnam Pharmaceutical Corporation (Vinapharm), the Services of Health of provinces and cities under the Center, branches, departments of health, pharmaceutical manufacturers, central and local hospitals.
b- The Services of Health of provinces and cities under the Center, Vinapharm, branches and departments of health should be responsible for disseminating and training principles of “Good storage practices” to entities placed directly under including hospital’s pharmacy departments, Research Institute and medical centers in the administrative division.
c- Entities themselves should expedite dissemination and training in principles of “Good storage practices” to their staff.
2.2 Registration procedures and inspection authorities
2.2.1 Inspection registration
Entities, after carrying out the inspection and evaluation on their own as having met “Good storage practices” standards, shall submit the inspection registration dossier on “Good storage practices” to the Ministry of Health (Drug Administration of Vietnam) Registration dossier consists of:
- Registration of inspection of “Good storage practices” (Attached form No.3-GSP/MB)
- Programs, subject matters and results from the training in “Good storage practices” at the establishment.
- Organization chart and payroll
- Map of geographical positions and design of stores
- Environmental assurance acknowledgement issued by competent agencies.
- List of storage facilities.
- List of stored product categories
- Minutes of self-inspection of “Good storage practices”
Registration dossier is made into 03 sets and sent to Drug Administration of Vietnam.
2.2.2 Inspection registration dossier:
Inspection registration dossier consists of:
- Registration of re-inspection of “Good storage practices”
- Reports on changes in payroll, organization, workshops of the entity during two years of implementing “Good storage practices”.
- Summary reports on programs, subject matters and results from the training in “Good storage practices” during two years.
- Reports on inspection and assessment by the entity itself in the most recent phase for the implementation of “Good storage practices”.
2.2.3 Inspection and certification authorities
Drug Administration of Vietnam shall be responsible for receiving and appraising the inspection registration dossier, formulating the Inspectorate and certifying the entity as having met “Good storage practices” standards.
2.2.3.1 Credentials of inspectors
- University degree and over, experienced in pharmaceutical management in general and pharmaceutical product storage in particular.
- Honest and objective
- Having systematical working method, strong determination in complying strictly with statutes and regulations, ability to quickly detect shortcomings and to advance convincing solutions helping entities to cope with them.
2.2.3.2 Appraisal of inspection registration dossier on “Good storage practices”
After receiving the inspection registration dossier on “Good storage practices” submitted by the entity according to the regulation (section 2.2.1 and 2.2.2), Drug administration of Vietnam shall coordinate with specialists of relevant agencies for appraisal (inclusive of appraisal record). Within 30 days since all eligible documents, Drug Administration of Vietnam shall issue a decision to establish the Inspectorate to inspect “Good storage practices” at the establishment. If the dossier fails to meet requirements, Drug Administration of Vietnam shall issue an official dispatch specifying the reason.
2.2.3.3 Inspection and certification authorities
- Participants of the Inspectorate: Drug administration of Vietnam and the Services of Health of provinces and cities under the Center where the enterprise is headquartered.
- Inspection record shall be signed for confirmation by representative of entity and head of the Inspectorate and made into three (03) copies: the entity, Drug administration of Vietnam and the Services of Health of provinces and cities under the Center each keeps one (01) copy.
After inspection, if the entity is found to have met the principle of “Good storage practices”, the Ministry of Health (Drug administration of Vietnam) shall grant the Certificate of good storage practices. The Certificate of good storage practices shall have a validity of two years since the day of signing. Three months before the Certificate becomes invalid, the entity should prepare procedures of registration for re-inspection according to Sections 2.2.2, Point 2.2, Item 2, Part III hereof. If the standards are met under the regulations, the entity shall be re-granted the Certificate.
The inspection of “Good storage practices” shall be carried out every two years unless otherwise required unexpectedly by the Ministry of Health (Drug administration of Vietnam).
